A copy of an application form is available from the hospital or treatment centre from which the patient is receiving treatment or by calling the Tribunal's Office.
An application for a review is to be made in writing to the Registrar, Mental Health Tribunal.
The hospital or treatment centre can assist you to make an application or to forward that application to the Tribunal.
The Tribunal will accept faxed copies of the Application for Review.
