What information are you required to provide to the Tribunal when an order is made or renewed?

You must notify the Registrar of the Tribunal in writing within 48 hours of :

 
In the case of information being withheld you must also state the reasons why the inclusion of the information would have an adverse effect upon the patient's treatment.

These notifications should be made on Form A (48 hour notification to Tribunal) which can be faxed to the Tribunal on 62 334509.
